Active Leeds accepts referrals from a range of Health and Care Professionals such as Physiotherapists, GP’s, Nurses, Community Support Workers, Social Workers, Health Support Services and Rehabilitation Teams such as cardiac, falls, stroke, diabetes, respiratory etc.

There is an inclusion criteria to ensure the safety of referrals and staff working on the programmes. We provide doubling staffing at a number of our interventions, however we don’t have the ability to provide one to one support for an extended time. The aim is for referrals to manage their own health conditions and their own activity levels, once they have received guidance, sometimes this will mean self-guided activities that are impossible measure accurately.
For people with no internet access, there are printed versions of all activities and this can be supported with phone calls or where appropriate outdoor activities.
