<b>Cardiac Rehabilitation – In partnership with Leeds Community Healthcare</b></p><p>
<b><b> </b></b></p><p>Active Leeds work in partnership with Leeds Community Healthcare to provide support to referral participants to return to physical activity following a cardiac event. Referrals will work with a qualified BACPR health and wellbeing coach for a 10 week period.<br></p><p>Support includes a pre and post programme health assessment, health coaching support to discuss individual preferences, goal setting and barriers, support into an activity of the individual’s choice and planning for long term exercise adherence.</p><p>All participants must be referred onto the programme by the Leeds Community Healthcare Cardiac team.<br></p><p><strong><strong>Active Leeds Keep Moving </strong></strong><br></p><p style="text-align:justify;">This is a structured physical activity programme for people with health conditions. The programme is perfect for beginners and delivered in Active Leeds leisure centres.
